Change the name of a hospital

Notes                

• All current licence holders must sign the application
• If a licence holder is deceased please contact the Board
• Please ensure the Board receives your application at least 14 days before the next Board meeting
• If the Board approves the change of name application, after the meeting of the Board:
  • Your hospital licence information is updated on our online Hospital Register
  • You will be sent a letter of acknowledgement noting that the Board has approved the change of name
  • You will be sent a new Veterinary Hospital Licence
